LVN Home Health Shift Care Nurse (Part-Time)

Schedule: Wednesday, Thursday, Friday, and Saturday 0000-0800 PST

Pay range is based off amount of experience

Responsibilities

• Provide comprehensive nursing care during home health shifts, including patient assessments, vital signs monitoring, and medication administration using EMR (Electronic Medical Records). Experience in Well Sky (Kinnser) is a plus.
• Administer injections, catheterizations, and perform sterile procedures following aseptic techniques to ensure patient safety and comfort.
• Experience in complex cases involving acute pain management, infection control, non- invasive ventilator care, tube feeding, and wound care such as suturing and catheter care is a plus.
• Collaborate with multidisciplinary teams to develop and implement individualized care plans that include discharge planning and case management strategies.
• Educate patients and their families on health coaching topics such as medication management, nutrition, mobility assistance (Hoyer lift), and chronic disease management including cardiology, geriatrics, or behavioral health.
• Ensure compliance with HIPAA regulations by maintaining accurate medical documentation and medical records while safeguarding patient confidentiality.
• Knowledge in medication management, vital signs, wound care, and proper lifting techniques.

Qualifications

• Valid LVN License (Must be applicable in the state of California)
• Active Basic Life Support Certification
• Home health nursing: experience with long-term care facilities like assisted living or nursing homes is highly valued.
• Familiarity with medical terminology, anatomy knowledge, physiology concepts, and diagnostic evaluation techniques essential for thorough patient assessments.
• Proficiency in medical documentation using EMR/EHR systems such as Well Sky Kinnser; experience with ICD coding (ICD-9/ICD-10) is a plus.
• Strong interpersonal skills to work effectively with people of diverse backgrounds—including those with developmental disabilities—and provide compassionate care.
• Ability to work independently during shifts while maintaining excellent communication within the healthcare team
• Knowledge of infection control protocols and sterile processing techniques necessary for safe practice in home environments.
• Must have a valid CA Driver's License

Job Type: Part-time

Pay: $31.00 - $33.00 per hour
