Job Summary
Join our dedicated hospice team as a Per Diem On-Call Licensed Vocational Nurse (LVN) specializing in hospice care within Contra Costa County and surrounding areas. In this vital role, you will provide compassionate, patient-centered nursing care to individuals at the end of life, ensuring comfort, dignity, and support for both patients and their families. This flexible on-call position offers the opportunity to make a meaningful difference while working in a dynamic healthcare environment. You will respond promptly to patient needs, assess symptoms, and coordinate care plans, all while maintaining compliance with healthcare regulations and standards.
Compensation/Requirement for On Call:
- Must be available for 1 week for : Mon-Fri(5pm-8am) & Sat/Sun(all day)
- $700 stipend + per visit rate + mileage
- Must be able to travel ALL within Sonoma, Santa Rosa, and Marin
Responsibilities
- Respond to on-call requests promptly, providing expert triage and assessment of patient needs in a hospice setting.
- Conduct comprehensive patient assessments including vital signs, symptom management, and physical examinations.
- Administer medications, including injections and IV infusions, following strict protocols and documentation standards.
- Collaborate with interdisciplinary teams to develop and update individualized care plans that prioritize comfort and quality of life.
- Educate patients and families on disease progression, medication management, and comfort measures such as tube feeding or catheter care.
- Maintain accurate medical documentation within Electronic Health Record (EHR) systems like Epic or Cerner, ensuring HIPAA compliance.
- Provide guidance on emergency situations such as acute pain management or airway management, escalating as needed to higher levels of care like ICU or trauma centers.
Experience
- Valid LVN license with current state licensure required.
- Prior experience in hospice care or palliative nursing strongly preferred.
- Critical care experience including ICU or emergency medicine is highly desirable.
- Familiarity with EMR/EHR systems such as Wellsky/Kinnser
- Experience working with diverse populations including individuals with developmental disabilities, dementia, Alzheimer’s disease, or memory care needs.
- Background in home & community care settings including assisted living, nursing homes, or hospital environments such as Level I Trauma Centers or Level II Trauma Centers is advantageous.
- Skills in patient assessment, medication administration (including injections), IV insertion, venipuncture, catheterization, and sterile technique.
- Ability to handle complex cases involving acute pain management, infectious disease care, and end-of-life support with compassion and professionalism.
