LVN to rehab facility

U Matter Holistic Detox and Recovery Center that caters to individuals who are looking for alternative treatment options for recovery from substance use.

We are seeking a skilled and compassionate RN and LVN to join our team. The lead RN and per diem LVN will provide assessments, treatment planning, oversee documentation of other clinicians, after care planning for clients, train staff and collaborate with other healthcare professionals to ensure the highest quality of client care. The ideal candidate will have excellent clinical skills, experience working in a detox facility, a strong commitment to patient advocacy and a passion for making a difference in people's lives.

Experience in Addiction and mental Health Treatment facilities required.

Salary Range: DOE per diem/ biweekly

Responsibilities

• Conduct comprehensive patient assessments
• Work closely with Psychiatrist for coordination of care
• Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
• Maintain accurate and up-to-date patient records.
• Participate in quality improvement initiatives to enhance patient care outcomes.

Qualifications:

• Registered Nurse or LVN
• Proven experience in the field of substance use facilities & Detox
• Excellent clinical assessment and diagnostic skills.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced healthcare environment.
• Compassionate and caring with a focus on patient-centric care.
• Ability to work collaboratively as part of a multidisciplinary healthcare team
• Must be able to commute to La Crescenta
