Job Summary
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN), Register Nurse, Caregivers, to join our healthcare team. The ideal candidate will provide high-quality patient care, support medical staff, and ensure the well-being of patients in various settings including long-term care, assisted living, and acute care environments. Nurses will play a crucial role in patient assessment, medication administration, and collaborating with interdisciplinary teams to deliver exceptional healthcare services.
Duties
- Administer medications and treatments as prescribed by physicians.
- Conduct patient assessments and monitor vital signs.
- Provide direct patient care including tube feeding, catheter care, and wound management.
- Assist with IV infusion and medication administration.
- Collaborate with healthcare team members to develop and implement patient care plans.
- Document patient information accurately in EMR/EHR systems while adhering to HIPAA regulations.
- Support patients with activities of daily living (ADLs) and personal hygiene.
- Educate patients and families on health management and care plans.
- Participate in discharge planning and case management for patients transitioning to home or other facilities.
- Maintain a clean and safe environment while following infection control protocols.
Qualifications
- Current LVN, RN, license in the state of practice.
- Experience in critical care, long-term care, or assisted living preferred.
- Familiarity with Kinser, Oasis is a plus.
- Knowledge of ICD-9/ICD-10 coding is desirable.
- Experience working with individuals with developmental disabilities or dementia care is an asset.
- Strong understanding of medical terminology, anatomy, physiology, and basic life support (BLS).
- Ability to perform phlebotomy, venipuncture, and specimen collection/processing.
- Excellent communication skills and ability to work collaboratively within a healthcare team.
