Lymphedema/Wound Care Physical Therapist / Occupational Therapist (Outpatient)

We’re seeking a passionate PT or OT with lymphedema/wound care experience to join our therapy team at a senior living community and bring specialized care directly to those who need it most.

Responsibilities

• Evaluate and treat patients with acute and chronic wounds, including surgical wounds, diabetic ulcers, venous/arterial ulcers, and pressure injuries.
• Develop and implement personalized care plans, collaborating with referring providers and interdisciplinary teams.
• Select appropriate dressings, monitor healing progress, and adjust interventions as needed.
• Educate patients and caregivers on wound prevention, mobility, and self-management.
• Travel to patient locations within your region and complete timely documentation.
• Serve as a clinical resource for wound management and skin integrity best practices.

Qualifications

• Licensed Physical Therapist (PT) or Occupational Therapist (OT) in CA.
• 1–2+ years of wound care or integumentary experience preferred.
• Strong clinical reasoning and ability to work independently in the field.
• Reliable transportation and valid driver’s license.
• Excellent communication and documentation skills

Job Types: Full-time, Part-time

Base Pay: $62.00 - $72.00 per hour
