What you will do:
We are looking for a MANAGER OF POPULATION HEALTH ANALYTICS who can help shape our vision and support our mission. Here is what the position will look like.
  • Supports the Director of Population Health in addressing operational needs, process implementation and inter-departmental activities while improving internal team workflows and operational efficiencies.
  • Leads cross-functional collaboration to ensure the successful execution of population health initiatives, utilizing technical tools and analytical insights to promote whole-person care and equitable outcomes.
  • Establishes and monitors population health performance metrics, ensuring accurate data collection, effective analysis, and timely reporting of population health outcomes.
  • Develops and standardizes advanced analytical methodologies, evaluation, and technical frameworks for population health strategies, including application of risk stratification tools and methods to identify rising risk populations that guide team implementation.
  • Manages a team of population consultants to perform population-level analyses, translate findings into actionable insights, and collaborate with clinical subject matter experts in developing interventions that promote health equity and improve member outcomes.
  • Facilitates the operationalization of evidence-based strategies in health promotion, population stratification and management that is in line with the Department of Health Care Policy and Financing’s Population Health Management Framework.
  • Partners with internal departments and external partners to design and operationalize shared initiatives, serving as a liaison and facilitating communication and collaboration.
  • Oversees compliance with data reporting requirements, ensuring accurate data management and timely submission of analytic reports.
  • Prepares and presents program outcomes to internal leadership and external audiences, translating findings into actionable insights.
  • Leads and develops a team of health professionals, fostering skill development while maintaining a collaborative and inclusive team culture that values diversity and innovation.
  • Raises issues or barriers to relevant team members in a timely manner and promote an action and solutions-oriented approach to complex problems with enterprise-wide impact
  • Manages vendor relationships for delegated population health programming, as appropriate, ensuring adherence to deliverables, evidence-based practices, and process standardization.
What you will bring:
Education: Bachelor’s degree in public health, health administration, or a related field required. Master’s degree highly preferred. A combination of education and experience can be substituted in lieu of a degree.
Experience: Minimum of 5 years’ experience in population health, public health or health plan operations, including at least 3 years’ prior experience in leading a team focused in utilizing data analytics to drive program development, monitor performance, and continuous improvement. Experience with predictive modeling, risk stratification methodologies, claims data analysis, and outcome evaluation techniques required. Strong background in statistics or epidemiology highly preferred. Prior experience with Medicaid and Child Health Plan Plus populations and experience with Chronic Illness and Disability Payment System (CDPS) and Johns Hopkins ACG model preferred. Experience in strategic planning, organizational change management, and cross-functional leadership preferred.
Knowledge, Skills, and Abilities: Possesses advanced leadership skills, a deep understanding of public and population health concepts, and a proven track record in developing and mentoring a team. Exceptional leadership and strategic thinking abilities to lead, mentor, and manage a team with influencing outcomes across organizational levels. Comprehensive knowledge of population health policies, trends, and best practices for implementing population health strategies that align with organizational goals and improve health outcomes. Effective communication and presentation, abilities, including experience representing organization in external forums. Experience working with analytic tools including MS SQL, Power BI, Excel (advanced) and complex datasets, including healthcare claims and publicly available data (e.g., American Community Survey). Demonstrated ability to work collaboratively in a team and cross-functional environment while driving alignment and accountability across departments. Ability to manage and prioritize multiple complex projects and deadlines, anticipate and mitigate risks. Strong analytical skills, including cost-effectiveness analysis, and ROI evaluation, to ensure efficient and effective use of resources in population health programs. Exceptional organizational and project management skills with experience in scaling operations and managing vendor relationships. Demonstrates support for the company’s mission, vision and values. Position requires excellent written and verbal communication skills. Must be proficient in Microsoft office applications (Word, Excel, PowerPoint, Outlook, SharePoint) with ability to work with a variety of project management tools (e.g., Smartsheet’s). Required ability to manage multiple priorities and projects with tight deadlines.
Licenses/Certifications: A valid driver's license and proof of current auto insurance will be required for any position requiring driving.

Pay, Perks and Benefits at Colorado Access:
The compensation for this position is $105,865.00 - $122,100.00 annually. Colorado Access has provided a compensation range that represents its good faith estimate of what Colorado Access may pay for the position at the time of posting. Colorado Access may ultimately pay more or less than the posted compensation range. The salary offered to the selected candidate will be determined based on factors such as the qualifications of the selected candidate, departmental budget availability, internal salary equity considerations, and available market information, but not based on a candidate’s sex or any other protected status.

Where you will work:
This position will be a hybrid model work environment, a blend of ‘In-Office’ and ‘Remote.’
