Job Summary

The Manager is responsible for advancing organizational goals through the development and oversight of population health programs, payor contract strategy, and data-driven performance management. This role partners with clinical, operational, and external stakeholders to optimize value-based care initiatives, strengthen financial and operational outcomes, and support innovation that improves quality, reduces cost, and enhances patient experience.

Job Specific Duties and Responsibilities

  • Lead population health and value-based care initiatives by partnering with clinical and operational leaders to leverage strategies and data analytics that improve outcomes, reduce costs, and advance organizational goals.
  • Manage payor strategy and contract performance by interfacing with payors to align, monitor, and optimize agreements that advance organizational priorities and maximize shared savings opportunities.
  • Oversee program evaluation and innovation through the design of pilots, proformas, and ROI analyses to support scalable, strategic initiatives.
  • Collaborate with internal and external stakeholders—including providers, payors, and community partners—to align strategies and advance population health objectives.
  • Manage departmental operations by overseeing daily functions, addressing operational challenges, and resolving personnel issues as they arise.
  • Ensure strong financial performance and forecasting by analyzing trends that inform strategic and operational growth and decision-making.
  • Evaluate and strengthen data systems and analytics by developing frameworks for data governance, key performance indicators, and actionable reporting to support decision-making and operational improvement.

Organizational Responsibilities

In addition to the duties and responsibilities listed above, leaders are expected to support and uphold the Summit Pacific mission, vision and values; comply with policies, procedures, and regulatory requirements; and conduct themselves in an ethical, professional, respectful, and collaborative manner.

Required Education and Experience

  • Master’s degree or equivalent; BSN preferred.
  • Two years of supervisory experience.
  • Two years of related clinical work experience, including population health, value-based contracting/care, and care coordination/care management.

Required Licenses, Certifications and/or Registrations

Basic Life Support (BLS) required within 60 days of hire.

Required Knowledge, Skills, Abilities

  • Knowledge of population health practices, value-based contracting, and related care coordination/care management practices.
  • Ability to cultivate effective relationships and work in collaboration with medical and administrative staff, patients, and the public. Strong leadership, facilitation, delegation, and coaching skills.
  • Proficient in the use of current technology, including Microsoft Office products. Ability to learn and effectively use electronic medical records and other systems and equipment.
  • Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience. Ability to maintain accurate documentation and records in a timely manner.
  • Demonstrated skill in process improvement, planning, organizing, and change management.
  • Ability to work independently performing a wide variety of assignments that require the use of independent judgment, systems-thinking, problem-solving, organization, and prioritization skills.
