Home Care Marketer / Community Liaison (OPWDD)

Salary Range: $55,000 – $75,000 + commission (commensurate with experience)

We are a growing home care agency seeking a motivated and relationship-driven Home Care Marketing Liaison to develop referral relationships for pediatric and OPWDD-funded home care services. This role focuses on building partnerships with care managers, and to do community outreach to community organizations, healthcare providers, and family support networks to connect children and families with in-home services.

This is a field-based position ideal for someone who enjoys in-person outreach, relationship building, and serving as the agency’s representative within the community.


Key Responsibilities

  • Build and maintain referral relationships focused on pediatric and OPWDD-funded home care services

  • Identify and proactively reach out to individuals and families who may qualify for OPWDD services, and provide guidance on how to begin the eligibility and enrollment process

  • Conduct regular in-person outreach to care managers, Care Coordination Organizations (CCOs), community organizations, schools, therapy providers, and other referral sources

  • Educate referral partners and families on agency services, eligibility requirements, and the intake process

  • Serve as the primary point of contact for referral sources and support ongoing relationship management

  • Coordinate closely with intake to facilitate timely starts of care

  • Attend community events, resource fairs, and networking opportunities relevant to pediatric and OPWDD populations

  • Represent the public relations the agency professionally and ethically in all community interactions

Required Qualifications

  • Strong interpersonal and communication skills

  • Comfort with in-person outreach and community-based work

  • Highly organized with strong follow-through and time-management skills

  • Ability to work independently and manage a defined territory

  • Professional demeanor and ability to represent the agency externally

  • Valid driver’s license and reliable transportation

Preferred Qualifications

  • Experience in home care, healthcare marketing, community outreach, or care coordination

  • Familiarity with OPWDD services, pediatric populations, or Medicaid-funded programs

  • Existing relationships with care managers, CCOs, or community organizations

  • Knowledge of referral-based healthcare systems

  • Multilingual abilities or cultural familiarity with the communities served
