Job Summary
We are seeking a dedicated and compassionate Master Social Worker to join our dynamic healthcare team. In this vital role, you will provide expert social work services across various clinical settings, including inpatient, outpatient, hospice, and community care environments. Your primary focus will be to support patients and their families through comprehensive assessments, individualized care planning, and therapeutic interventions. You will play a key role in facilitating positive health outcomes by integrating behavioral health strategies with medical care, ensuring a holistic approach to patient well-being. Responsibilities

  • Conduct thorough patient assessments to identify social determinants affecting health and recovery
  • Develop personalized care plans that incorporate behavioral therapy, crisis intervention, and case management strategies
  • Provide individual and group counseling sessions utilizing techniques such as motivational interviewing, cognitive-behavioral therapy, and psychotherapy
  • Collaborate with medical teams to coordinate patient care and ensure compliance with HIPAA regulations for medical documentation and records
  • Facilitate family meetings and provide child & family counseling to support emotional well-being and coping skills
  • Manage intake processes for new patients, ensuring accurate data collection and comprehensive clinical documentation
  • Assist with discharge planning, post-acute care coordination, and community resource linkage for seamless patient transitions

Requirements

  • Master’s degree in Social Work (MSW) from an accredited program; licensure as a Licensed Clinical Social Worker (LCSW) or equivalent required
  • Proven experience working in hospital settings, hospice care, or outpatient clinics with a focus on behavioral health or mental health counseling
  • Strong knowledge of medical terminology, ICD coding (ICD-10/ICD-9), CPT coding, and electronic medical record (EMR) systems
  • Expertise in crisis management, diagnostic evaluation, addiction counseling, dementia care, Alzheimer’s care, PTSD care, autism spectrum disorder interventions, and working with individuals with disabilities or developmental disabilities
  • Familiarity with HIPAA compliance standards for safeguarding patient information and medical records management
  • Excellent communication skills for conducting intake interviews, clinical counseling sessions, group therapy groups, and family interventions
  • Ability to work effectively within interdisciplinary teams in inpatient, outpatient, nursing home, assisted living or home & community care environments

Pay: $60,000.00 - $65,000.00 per year
