MCIS Overnight Case Manager

The Child and Family Guidance Center is currently seeking a full time Overnight Case Manager in our Bridgeport Mobile Crisis Intervention Services group.

Working an overnight shift Monday through Friday from 9PM to 6AM the Overnight Case Manager will assist MCIS clinicians meet the clinical needs of the families they work with by securing appropriate and necessary services to children ages zero to 18 experiencing a behavioral or mental health crisis. MCIS case manager’s goal is to serve children in their homes and communities to reduce the number of visits to hospital Emergency Departments and divert them from hospitalization if a lower level of care is a safe, effective alternative.

Salary Range: 55,000 to 60,000

The selected individual will be responsible for but not limited to the following Essential Functions:

  • Assist families in obtaining and securing appropriate necessary services. Transportation, translation, advocacy, educational support, recreational and social support and other services as needed.
  • Works as a team with MCIS clinicians to help children and their families by maximizing the smooth linkage to follow up services.
  • Provide mobile crisis services with the MCIS clinician to a child and the family as needed to address child and family social and psychiatric problems that arise in the home, school or community.
  • Provide outreach to other community agencies, school, courts and individuals as deemed appropriate.
  • Participate in weekly supervision and attend case conferences and team meetings.
  • Maintain accurate records, assist in data collection, and contribute to quality assurance efforts.
  • Manage screening of 211 calls and Outpatient ECC calls as needed

Qualifications:

  • Bachelor’s degree in Social Work, B.S or B.A educational level.
  • Must possess at least 2 years’ experience working with children and families with emotional and behavioral difficulties and their families
  • Knowledge about child development and parenting.
  • Must have a driver’s license with access to a car and able to travel to travel within the greater Bridgeport and or Norwalk areas as needed.
  • Bi-Lingual Spanish Speaker is preferred
  • Ability to work collaboratively with others while meeting deadlines
  • Able to work in an unstructured environment under pressure with intense emotional situations.
