Our Family Practice Physician/Internal Medicine Physician is responsible for diagnosing and treating a variety of diseases and injuries as a Care Team member in the medical clinic using best practices as modeled in a Patient Centered Medical Home (PCMH) setting; by providing comprehensive, outpatient family practice medical services for patients on a continuing basis. The Family Practice Physician is a member of the multidisciplinary clinic team that has delegated powers and responsibilities to provide effective patient-centered care.

Due to real and potential conflicts of interest that exist when HOPES Behavioral Health providers render services to their coworkers, it is HOPES’ policy (Policy HR0005) that HOPES employees may not access HOPES Behavioral Health Services. For the same reason, current Behavioral Health patients/clients cannot be employed by HOPES. Accordingly, you hereby acknowledge and understand that if you are receiving services from a HOPES Behavioral Health provider at the time you are offered and accept employment with HOPES, you have the option of rescinding your acceptance of employment or finding a new behavioral health provider outside of HOPES before starting employment with HOPES.

Graduate of an accredited medical school, residency program and boardcertified/eligible in family practice or internal medicine

Active state Pharmacy and DEA license, including controlled substance prescribing upon onboarding.

Valid BLS required prior to hire, per HRSA compliance.

Knowledge of the essential functions, practices and procedures of a medical clinic and office.

Knowledge of CPT and ICD10CM coding.

Knowledge of and ability to diagnose and treat a variety of diseases and injuries in Family Practice primary care setting, acting as part of a Care Team.

Knowledge of skills related to the ability to treat patients in an immediate care Family Practice setting (acute illness and injury appropriate for outpatient level of care).

Skill in operating personal computer utilizing electronic medical records, word processing, databases and email.

Skill in operating various office equipment: calculator, copy/scanning machine, fax machine, and telephone.

Skill in operating and interpreting various outpatient medical office equipment.

Ability to maintain confidentiality of patient information , financial information and medical information.

Ability to read, write legibly, calculate mathematical figures and medication doses.

Ability to solve practical problems dealing with a variety of variables as a Care Team member.

Ability to exercise good judgment in appraising situations and making decisions.

Ability to work and interact effectively and positively with other staff members in a Care Team structure to build and to enhance teamwork in the clinics and across the HOPES organization.
