Job Description:

Are you organized, detail-oriented, and passionate about making a difference? We’re looking for an energetic MDS Coordinator to join our team and help ensure our residents receive the best care possible!

Schedule:

Monday–Friday
Day Shift

What You’ll Do:

Complete and coordinate resident assessments (MDS) accurately and on time
Work closely with nursing staff, therapy, social services, and physicians
Ensure care plans are individualized and up to date
Monitor Medicare/Medicaid documentation and reimbursement
Maintain compliance with state and federal regulations

Track due dates, schedules, and submission deadlines
Support quality measures and help improve resident outcomes
Communicate with families and team members when needed

What We’re Looking For:

RN/LPN license preferred
MDS experience is a plus (but we’ll help train the right person!)
Strong attention to detail
Great communication and teamwork skills
Compassion for residents and their families
Ability to stay organized and meet deadlines
