Job Title: MDS Clinical Assessment RN

Location: On the road. In-person assessments. Remote documentation. Massachusetts (Essex County and or Middlesex county.) North shore area to Somerville/Cambridge/Medford.

Employment Type: Full-Time

Rate: $215 per assessment completed. Mileage reimbursement at the standard IRS rate.

Start Date: ASAP

Shifts: Customizable schedules: Monday-Friday. In person assessments are scheduled at 9am, 11am, 1pm, 3pm. RN must have 12 open slots per week. Choose from flexible options, including 4 days per week. No weekends, no holidays, no overtime, and no on-call. Regular schedules are Monday - Friday.

Contract Length: 3-6 months

About the Role

LCG is partnering with Commonwealth Care Alliance (CCA) and is seeking a dedicated and compassionate Clinical Assessment RN to join the MDS Assessment Unit. This position plays a critical role in identifying the clinical and social needs of CCA members through in-person and telephonic assessments and ensuring timely, high-quality documentation that supports individualized care plans.

Working in a geographically assigned territory, you will conduct, and document comprehensive, face-to-face assessments using the Minimum Data Set (MDS) and supplemental tools, helping to identify unmet needs and enable the delivery of person-centered care.

Key Responsibilities

  • Perform assessments: Conduct in-person MDS assessments using tools such as PHQ-9, EASI, AUDIT, and DAST.
  • Document with accuracy: Complete SOAP notes, MDS documentation, and care interactions in electronic health records (eCW, GuidingCare) within 72 hours; correct flagged assessments within 48 hours.
  • Ensure quality of care: Escalate urgent member needs to supervisory staff, maintain confidentiality, and uphold organizational policies and procedures.
  • Collaborate effectively: Communicate clearly with care team members to support coordinated, person-centered care delivery.
  • Leverage technology: · Use EMR, care management platforms, and claims platforms to collect data, document member interactions, organize information, and communicate with your team. Manage assessment activities through the calendar and track the status of completion.

Required Qualifications

  • Associate Degree in Nursing (ADN)
  • Active RN license in good standing (Massachusetts)
  • Minimum 1 year of clinical experience
  • Proficiency with electronic medical records (EMR)
  • Strong computer skills and problem-solving abilities
  • Driver’s license, vehicle, and ability to travel across Massachusetts for home visits
  • Proficiency in English; reliable internet connection for remote work
  • Experience with Microsoft Office (Excel, Outlook, Teams)

Preferred Qualifications

  • Bachelor’s Degree in Nursing (BSN)
  • Experience with eCW and GuidingCare
  • Previous MDS-HC experience
  • Bilingual skills (Spanish, Portuguese, Vietnamese, Russian, etc.)
