As the MDS Nursing Coordinator at Wesley Manor, you are responsible for the accuracy and completion of the MDS Assessments. The MDS Nurse plays a crucial role in ensuring quality care and regulatory compliance within our community.

In This Role You Will:

  • Completes computerized or written MDS forms and quarterly summaries within required time frame. Assures completion of the MDS by each department within the required time frame. Update care plans as needed.
  • Transmits completed MDS forms to appropriate state and Federal agencies at required time frame, as required by regulation.
  • Helps maintain communication about Med A patients between therapy staff and Wesley Manor staff; (e.g., new admissions; admit and discharge dates; RUGS; care plan schedules; and occupational therapy, physical therapy, and speech therapy patient treatment times).
  • Informs residents/responsible parties of Medicare covered days and notifies them of their last covered day.
  • Updates MDS care plan schedules.
  • Writes and/or assists the director of nursing in writing resident care plans for each resident. Chairs the care plan meetings.
  • Keeps Medicare certifications up to date.
  • Collaborate with interdisciplinary teams to capture all Medicaid-reimbursable services and resident care needs
  • Monitor and manage Case Mix Index scores to optimize Medicaid reimbursement levels
  • Participate in Medicaid audits and surveys, ensuring documentation supports medical necessity and level of care determinations
  • Maintains certification to give Mantoux tests, CPR, and first aid.
  • Provide education and training to nursing staff on MDS assessment protocols, documentation requirements, and regulatory guidelines.
  • Teamwork and participation maintain an effective relationship with all departments and professional staff; and participates on facility committees as assigned.

Shift:

  • Monday - Friday 8:00 AM - 4:00 PM w/on-call on rotating weekends

Who You Are:

  • Graduate of an accredited school of nursing
  • We prefer RN but require LPN
  • Active LPN/RN License
  • Preferred: certification of MDS assessment (RAC-CT)
  • Experience in Long-Term Care
