Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will be responsible for coordinating the comprehensive assessment process for residents in long-term care facilities, ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments. Your expertise will support quality patient care, regulatory compliance, and reimbursement accuracy. Responsibilities
- Coordinate and facilitate the completion of MDS assessments in accordance with federal, state, and local regulations, including CMS guidelines.
- Collaborate with interdisciplinary teams such as nursing, therapy, social work, and medical staff to gather comprehensive clinical information.
- Review medical records, documentation, and clinical data to ensure accuracy and completeness of assessments.
- Utilize PointClickCare to document findings and submit required reports efficiently.
- Ensure compliance with HIPAA regulations by maintaining confidentiality of resident information during all phases of assessment and documentation review.
- Conduct ongoing education for staff on MDS updates, coding standards (ICD-10), and regulatory changes affecting documentation practices.
- Support quality improvement initiatives by analyzing data trends related to patient care and reimbursement metrics.
Requirements
- Proven experience with MDS coordination in inpatient or long-term care settings; experience in nursing homes.
- Strong knowledge of medical terminology, anatomy, physiology, and clinical documentation improvement strategies.
- Familiarity with PointClickCare and proficiency in navigating medical records is essential.
- Understanding of managed care processes including Medicare regulations, utilization review, utilization management, and discharge planning.
- Experience with ICD-10 coding and case management practices.
- Ability to interpret complex medical records accurately while adhering to HIPAA compliance requirements.
- Excellent communication skills to collaborate effectively with multidisciplinary teams across outpatient and inpatient settings including hospice care or emergency medicine environments.
