The MDS Coordinator is responsible for conducting the Resident Assessment Instrument (RAI) process toensure individual resident medical, psychosocial, emotional and physical needs are identified, and care plans are developed to assist residents in achieving their highest practicable level of function.

Responsibilities and Duties

  • Complete all RAI processes including MDS and CAAs for assigned residents.
  • Accurately perform Medicare, Annual, and Quarterly assessments according to required schedules.
  • Conduct thorough bedside assessments and resident interviews to ensure personalized, resident-centered care plans.
  • Support and educate nursing staff on accurate PointClickCare documentation.
  • Collaborate with residents, families, and interdisciplinary team members to create and adjust individualized care plans.
  • Evaluate and document resident progress toward care goals, addressing barriers and coordinating needed resources.
  • Participate in facility committees and maintain health record accuracy in alignment with RAI and care plans.
Requirements:

Qualifications and Skills

  • Registered Nurse
  • RAC-CT certification preferred (or willingness to obtain)
  • Excellent verbal and written communication skills with residents, families, all levels of staff and external customers.
  • Prefer Resident Assessment Coordinator (RAC) Certification.
  • Excellent verbal and written communication skills with residents, families, all levels of staff and external customers.
  • Must possess a high degree of personal accountability, responsibility and the ability to make independent decisions when circumstances warrant such action.
  • Compassionate leadership and the ability to inspire a culture of quality and accountability.
