Summary

We are seeking a dedicated and detail-oriented MDS (Minimum Data Set) Coordinator to join our team. In this vital role, you will lead the comprehensive assessment process for residents, ensuring accurate documentation and compliance with federal and state regulations. Your expertise will directly impact care planning, reimbursement, and quality improvement initiatives across our communities. This position offers an exciting opportunity to make a meaningful difference in patient care through clinical documentation improvement and utilization management.

This position comes with a starting negotiable salary of $82,000 based on experience and licensure. Candidate must have at least one-year MDS experience is required for this position.

The typical schedule for this opportunity is Monday through Friday from 9am to 5pm. Start and end times are flexible based on community need. This is a hybrid position working onsite at the community Three (3) days a week and Two (2) days a week remote at a home office. There are no On-Call responsibilities associated with this position.

Responsibilities

  • Coordinate and facilitate the completion of MDS assessments in accordance with CMS (Centers for Medicare & Medicaid Services) guidelines and timelines.
  • Collaborate with interdisciplinary teams including nursing, therapy, social services, and medical staff to gather accurate clinical data.
  • Review medical records, documentation, and coding (ICD-9/10, CPT) to ensure completeness, accuracy, and compliance with regulatory standards such as NCQA (National Committee for Quality Assurance).
  • Conduct thorough documentation reviews to support clinical decision-making, reimbursement accuracy, and quality metrics.
  • Utilize EMR (Electronic Medical Record) systems like Cerner or eClinicalWorks to input data efficiently while maintaining HIPAA compliance.
  • Lead efforts in clinical documentation improvement initiatives to optimize patient care records and coding accuracy.
  • Support discharge planning, hospice care coordination, and case management activities as needed within inpatient or outpatient settings.

Requirements

  • Valid nursing license in the state of Ohio, Registered Nurse (RN) Preferred.
  • Proven experience with MDS coordination in long-term care facilities or skilled nursing environments.
  • Strong knowledge of CMS regulations, Medicare requirements, and NCQA standards related to MDS and utilization review.
  • Familiarity with EMR/EHR systems such as Cerner, Athenahealth, or similar platforms.
  • Experience with ICD-10/ICD coding, CPT coding, and DRG (Diagnosis-Related Group) assignment processes.
  • Background in nursing or healthcare administration with critical care or hospital experience preferred.
  • Knowledge of healthcare terminology, anatomy, physiology, and medical records management.
  • Ability to interpret complex clinical data accurately while maintaining attention to detail.
  • Excellent communication skills to collaborate effectively across multidisciplinary teams in inpatient or outpatient settings including ICU or trauma centers.
