MDS RN or LPN Position

Ark Healthcare and Rehabilitation at Saint Camillus, located in the heart of Stamford, CT seeks a Full-Time, Monday thru Friday, Full-Time MDS RN or LPN Coordinator.

This EXCITING opportunity will afford fully customized training to the right candidate willing to learn this role.

Primary responsibilities.

• Contributes to the facility interdisciplinary RAI process while following RAI guidelines for completion, computer coding and electronic transmissions of the MDS

• Coordinates the completion of the resident assessment instrument: MDS, CATs, and CAAs, in accordance with current rules, regulations, and guidelines that govern the resident assessment

• Manages the overall process and tracking of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for services provided within the Center.

• Conduct concurrent MDS reviews to assure achievement of maximum allowable PDPM categories.

• Attends/participates in facility IDT meetings as directed

• Identifies changes in condition in conjunction with IDT and coordinates scheduling and completion of indicated assessments

• Integrate information from nursing, dietary, social services, restorative, rehabilitation, and physician services to ensure appropriate reimbursement.

Candidate will possess:

• Unencumbered State of CT RN or LPN License

• Experience with Point Click Care Preferred

• 1 year in Long Term Care

• Great attention to detail and great organizational skills
