Responsibilities

• Assist with patient care activities including vital signs monitoring, catheter care, tube feeding, and hygiene assistance.
• Support residents or patients in assisted living, nursing homes, hospitals, or home care environments by providing compassionate caregiving and ensuring comfort.
• Document medical information accurately using Electronic Medical Record (EMR) and Electronic Health Record (EHR) systems such as Epic or Cerner.
• Administer medications following prescribed protocols while adhering to HIPAA regulations to maintain patient confidentiality.
• Support individuals with developmental disabilities or dementia by implementing specialized memory and Alzheimer’s care techniques.
• Assist with medical procedures involving anatomy knowledge, physiology understanding, and medical terminology comprehension.
• Collaborate with healthcare teams to monitor patient progress, record vital signs, and report any changes in condition promptly.

Skills

• Proven experience working in assisted living facilities, nursing homes, hospitals, or home care environments.
• Strong knowledge of medical documentation practices and familiarity with EMR/EHR systems like Epic or Cerner.
• Competence in medication administration, catheter care, tube feeding, and vital signs measurement.
• Understanding of medical terminology, anatomy, physiology, and hospice or dementia care principles.
• Ability to work effectively with individuals with disabilities or developmental challenges in various settings.
• Excellent communication skills to coordinate with healthcare professionals and provide clear patient updates.
