The Medicaid & Transition Case Manager is responsible for the end-to-end administration of the SHARS billing process and for coordinating state-mandated transition planning for special education students. This role services as a vital bridge between students, families, and state agencies, ensuring that rural districts maximize Medicaid reimbursement while providing students with a clear roadmap for post secondary life.
Coordinate the SHARS (School Health and Related Services) program for four partner districts to ensure maximum allowable reimbursement.
Monitor and verify service logs, provider documentation, and Admission, Review, and Dismissal (ARD) records for billing accuracy.
Conduct formal Transition Interviews for special education students beginning at age 13 to prepare for the age 14 state mandate.
Maintain absolute confidentiality of student records under FERPA and employee information under HIPAA guidelines.
Travel regularly between the four rural campuses to provide on-site support and training for staff.
Provide Bilingual (English/Spanish) support for ARD meetings, parent interviews, and written correspondence to serve our emergent bilingual families.
