Position : Case Manager/ Check-Out Department
Full time : Available Monday through Friday from 6:45 a.m. to 5:30 p.m.
Education requirements : Registered Medical Assistant (5 + year experience preferred), or required work experience
Reports to: Clinical Manager
Summary : A Case Manager greets the patient after encounter with provider, processes new/refill prescriptions and orders for labs and diagnostic tests, initiates referrals, and schedules follow up appointments. Responsibilities also include telephone triage and follow up and management of clinical tasks in the EMR.
