Medical Assistant - Palliative Care, Full Time

Location: Inpatient Center
Position Type: Full Time, 1.0 FTE, Hourly, 40 Hours/Week
Schedule Requirements: Monday-Friday, Day Shift

Purpose of the Position
The Medical Assistant (MA) is a vital member of the Rainbow Palliative Care interdisciplinary team. This role supports patients with serious and chronic illness across care settings by providing clinical support, care coordination, and assisting with referral management.
Clinical Support & Documentation
  • Maintain accurate and timely patient records, including demographics, medical history, medications, and care updates.
  • Assist with admissions, electronic consents, and referral documentation.
  • Reconcile medications and support provider orders within MA scope of practice.
  • Ensure clinical documentation meets organizational and regulatory standards.
  • Communicate visit notes and care updates to primary care providers, specialists, and care partners.
Patient Communication & Care Coordination
  • Triage incoming calls, addressing needs when appropriate or escalating to RN/NP.
  • Serve as a primary point of contact for patients, caregivers, and families.
  • Coordinate referrals, follow-up care, labs, imaging, and consults.
  • Order and follow up on DME and medical supplies.
  • Schedule NP visits as needed, including for changes in patient status.
  • Collaborate closely with interdisciplinary team members and external providers.
Follow-Up & Task Management
  • Track and close pending clinical tasks.
  • Communicate provider recommendations and follow up on ordered services.
  • Support continuity of care across settings.
Qualifications
  • High School Diploma or equivalent.
  • Licensed or Certified Medical Assistant.
  • Minimum of 1 year of MA experience (clinical setting preferred).
  • Proficiency with electronic medical record systems and Microsoft Office tools.
  • Valid Wisconsin driver’s license and proof of current auto insurance.
  • Experience in palliative care, hospice, geriatrics, or chronic disease management preferred.
  • Strong organizational, prioritization, and critical-thinking skills.
  • Compassionate, patient-centered approach.
  • Ability to assess urgency and adapt in a fast-paced, community-based environment.
  • Ability to maintain professionalism, confidentiality, and integrity.
  • Effective, courteous communication with patients, families, and colleagues.
