Position Overview:
The Telehealth Medical Provider delivers comprehensive, patient-centered care through virtual platforms. This role involves conducting telemedicine visits to assess, diagnose, and manage acute and chronic conditions while promoting preventive health and wellness. The provider collaborates with interdisciplinary teams to ensure continuity of care, documentation accuracy, and adherence to clinical and regulatory standards. This position reports to the Director of Clinical Services.
General Responsibilities (including, but not limited to):
- Ensure all medical services align with the Agency’s mission & vision.
- Conduct virtual visits for routine, follow-up, and urgent care needs.
- Develop and manage individualized treatment plans.
- Coordinate care with specialists, behavioral health providers, and care managers.
- Review labs, diagnostics, and medication management remotely.
- Maintain detailed and timely electronic health record (EHR) documentation.
- Provide patient education and promote self-management of chronic conditions.
- Provide medical feedback via thorough medical chart reviews relative to patients’ continuity of care, such as preventive and cancer screenings, labs, etc.
- Facilitate comprehensive medical chart reviews as a means of general supervision of patients.
- Uphold the agency’s protocols, policies & procedures to ensure compliance with treatment planning and regulatory requirements.
- Perform all other related duties as assigned.
Requirements and Qualifications:
- Current, unrestricted license and certification to practice medicine in the State of NY; multi-state licensure with preference of NY, NJ, and IMLC. MD or DO.
- Board-certified in Family Medicine, Internal Medicine, or related specialty.
- Minimum of 1–2 years of clinical experience (telehealth experience preferred).
- Strong communication, technology, and patient engagement skills.
- Reliable connectivity & internet access.

Pay: $175,000.00 per year
