Medical Doctor - Family Practice (Punawai Clinic)

Primary Purpose:


As a leader of the Patient Centered Medical Home Team, this position will provide healthcare to adult patients by leading a care management team using the patient-centered medical home team model. This person will also assist in clinic teaching and fulfill administrative responsibilities.


The person in this position is a member of the Patient Centered Health Care Home team at KPHC where each team member plays an integral role in providing patient-centered health care. This position is primarily located at the Punawai Clinic but may be scheduled any other KPHC clinic based on organizational needs.


Essential Duties:

  • Practice and provide care in accordance with acceptable standards of Family Medicine or Internal Medicine, the Clinical Practice Guidelines of KPHC, and other written policies and standing orders of KPHC.
  • Provide comprehensive patient care with the ability to consult and/or refer to other health care providers/services/agencies as necessary.
  • Order and interpret laboratory tests, studies and ancillary services.
  • Prescribe medical treatment and clinical drugs to patients.
  • Perform approved therapeutic procedures.
  • Dispense medications from within clinic supply in accordance with legal and clinic standards.
  • Maintain charting in accordance with legal and clinic standards.
  • Instruct and advise patients regarding preventive health care measures to promote physical and mental well-being and to promote early detection, diagnosis and management of medical conditions.
  • May be required to supervise Physician's Assistants or other staff as assigned.
  • Review and manage on a regular basis chronic care cases that require ongoing medical attention.
  • Provide/promote "informed consent" and knowledgeable participation of clients in their care and in preventive care.
  • Maintain confidentiality of all patient information according to federal guidelines and regulations.
  • Assist in the resolution of complaints, requests and inquiries from patients.
  • Provide health education/consultation to clients, families, other professionals and the community as appropriate.
  • Collaborate in planning and participate in teaching paraprofessionals, medical students, residents, nursing students and other health care professionals as assigned.
  • Expected to attend All-Staff Meetings bi-monthly and other staff and professional meetings as required.
  • Participate in health center and professional quality assurance plans/activities, data collection and analysis.
  • Participate in monthly peer review, support staff evaluation, case conferences, hospital committees and other activities as required.
  • Assist in evaluating and revising Clinical Practice Guidelines for practice in the clinic and in other practice policies and procedures.
  • Provide after-hours call coverage shared with other members of the adult medicine team.
  • May be required to provide outpatient care at one or more KPHC sites.
  • Communicate concerns and suggestions to the Medical Director, Clinical Operations Director and to other medical team providers verbally and in writing, where appropriate.

Regular and reliable on-site attendance is required.


Other Duties:

Perform other related duties as assigned.


Minimum Qualification Requirements:


Education:

  • Medical or Osteopathic doctorate degree from an accredited medical school required.
  • Completion of residency in Family Medicine or Internal Medicine at an accredited US residency program required.
  • Required to participate in continuing education programs and maintain current BLS certification.


Licensing/Certification

  • Unrestricted license to practice medicine in the State of Hawaii required
  • Board Eligibility or Board Certification in Family Medicine or Internal Medicine required
  • Current Basic Life Support certification required.
  • Current DEA and Hawaii State Narcotics registration required.
  • Must be eligible for credentialing with Quest and the majority of other third party payers with whom KPHC participates.


Skills/Knowledge:

  • Must be able to work with multi-ethnic and multi-cultural staff and clients.
  • Knowledge of and ability to apply professional medical principles, procedures, and techniques.
  • Thorough knowledge of pharmacological agents used in patient treatment.
  • Effective verbal and written communication skills along with proper telephone etiquette.
  • Perform in a tactful and professional manner.
