Primary Function : The Medical Services Nurse Coordinator will oversee the clinical and administrative aspects of the serious illness program. This role requires leadership in coordinating care between the interdisciplinary team, which includes physicians and nurse practitioners (NPs), to ensure patients with serious illnesses receive compassionate and comprehensive care in their homes or other community settings.

Qualifications :
1. Registered Nurse (RN) licensed in the state.
2. Minimum of 2 years of experience in palliative care, hospice, or a related healthcare field.
3. Experience in working with interdisciplinary teams and coordinating care across various settings.
4. Strong communication, organizational, and leadership skills.
Job Duties :
1. Care Coordination: Communicates with other members of the interdisciplinary team to ensure timely, effective, and compassionate care is provided to all patients.
2. Direct Patient Care: Performs (and documents) in-person patient assessments in patient homes and long-term care facilities, assists patients with telehealth visits with team physicians and NPs, and provides phone support for patients and families.
3. Administrative Duties: Coordinates the day-to-day administrative functions of the serious illness program, ensuring patient records, billing, and scheduling are completed efficiently. Assists with patient scheduling, prior authorizations, communication with referring practitioners and other administrative support for the team.
4. Team Collaboration: Works with team members, including physicians and NPs, to address the medical needs of patients and their families.
5. Patient Advocacy: Serves as a liaison between patients, their families, and the healthcare team to ensure patient goals and preferences are honored.
6. Quality Improvement: Contributes to the ongoing evaluation of program outcomes and the implementation of best practices to improve the quality of care.
7. Education & Training: Works with CMO/Medical Director to develop and implement a comprehensive training program for all team members to enhance their knowledge and skills in palliative care.
8. Attends required in-services on an annual basis:

10. Maintains confidentiality Protected Health Information (PHI) at all times.
11. Participates in agency sponsored in-service training.
12. Participates on agency committees as needed.
13. Performs job duties in accordance with all agency policies, procedures, and professional and community standards.
14. Abides by the agency’s Professional Code of Conduct.

Regular travel to patient homes and community settings.
Status and Hours of Work :
This position is classified as exempt. Usual working hours are 8 am to 5 pm, Monday through Friday, or as needed to perform duties. This position is not eligible for overtime pay.
This position has the flexibility to telework as approved by the supervisor.
