Position Summary
The Hospice Medical Social Worker provides emotional, social, and financial support to patients and their families based on assessed needs. This role serves as a liaison for coordinating community resources, participates in bereavement and educational programs, and offers consultation to staff, volunteers, and the community as needed.
Key Responsibilities

Complete intake and referral coordination to ensure timely patient admissions
- Conduct psychosocial assessments within 7 days of admission
- Develop and maintain individualized Social Work Plans of Care
- Provide counseling and support related to death, dying, and bereavement
- Assist patients and families with accessing community resources and referrals
- Evaluate insurance coverage and assist with available options
- Maintain a current directory of community resources
- Collaborate with the interdisciplinary team to support patient care
- Attend weekly team meetings and participate in care planning
- Maintain timely, accurate documentation (within 24 hours)
- Log visits and mileage accurately
- Provide on-call support for crisis intervention as needed
- Participate in discharge planning and care transitions
- Engage in community outreach and education efforts
- Complete required continuing education (minimum 12 hours annually)
- Follow all agency policies, procedures, and regulatory requirements
Qualifications

Bachelor’s or Master’s degree in Social Work or Sociology from an accredited institution
- Current Social Worker license/certification in the State of Oklahoma
- Minimum of one (1) year of community or in-home care experience preferred
- Strong verbal and written communication skills
- Understanding of hospice philosophy and the dying process
- Valid driver’s license, reliable transportation, and active auto insurance
- Ability to travel throughout the service area
- Must have a functional cell phone for work-related communication
