Medical Social Worker

Position Summary

The Hospice Medical Social Worker provides emotional, social, and financial support to patients and their families based on assessed needs. This role serves as a liaison for coordinating community resources, participates in bereavement and educational programs, and offers consultation to staff, volunteers, and the community as needed.

Key Responsibilities

• Complete intake and referral coordination to ensure timely patient admissions
• Conduct psychosocial assessments within 7 days of admission
• Develop and maintain individualized Social Work Plans of Care
• Provide counseling and support related to death, dying, and bereavement
• Assist patients and families with accessing community resources and referrals
• Evaluate insurance coverage and assist with available options
• Maintain a current directory of community resources
• Collaborate with the interdisciplinary team to support patient care
• Attend weekly team meetings and participate in care planning
• Maintain timely, accurate documentation (within 24 hours)
• Log visits and mileage accurately
• Provide on-call support for crisis intervention as needed
• Participate in discharge planning and care transitions
• Engage in community outreach and education efforts
• Complete required continuing education (minimum 12 hours annually)
• Follow all agency policies, procedures, and regulatory requirements

Qualifications

• Bachelor’s or Master’s degree in Social Work or Sociology from an accredited institution
• Current Social Worker license/certification in the State of Oklahoma
• Minimum of one (1) year of community or in-home care experience preferred
• Strong verbal and written communication skills
• Understanding of hospice philosophy and the dying process
• Valid driver’s license, reliable transportation, and active auto insurance
• Ability to travel throughout the service area
• Must have a functional cell phone for work-related communication
