Medical Social Worker - Home Health

Job Summary
We are seeking a compassionate and dedicated Medical Social Worker to join our home health team. In this vital role, you will provide comprehensive social work services to patients and their families, supporting their emotional, psychological, and social needs during recovery and ongoing care. Your expertise will help enhance patient outcomes by addressing barriers to care, coordinating community resources, and delivering personalized counseling. Responsibilities

  • Conduct thorough patient assessments to evaluate social, emotional, and environmental factors impacting health and recovery.
  • Develop individualized care plans that incorporate counseling, resource coordination, and support strategies aligned with medical treatment goals.
  • Provide clinical counseling services including individual therapy, group therapy, behavioral therapy, motivational interviewing, and crisis intervention.
  • Collaborate with interdisciplinary teams to ensure holistic patient care across inpatient, outpatient, home health, hospice, assisted living, and community settings.
  • Manage medical documentation accurately within electronic medical record (EMR) systems while adhering to HIPAA regulations for patient privacy and confidentiality.
  • Facilitate intake processes by gathering comprehensive social histories and conducting diagnostic evaluations for diverse populations including children with developmental disabilities or autism, seniors with dementia or Alzheimer’s disease, and individuals with PTSD or addiction issues.
  • Coordinate community resources such as mental health programs, hospice services, special education supports, or disability accommodations to optimize patient well-being.

Skills

  • Extensive experience in child & family counseling, behavioral health therapy, and clinical research methodologies.
  • Strong background in hospital experience with knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and medical records management.
  • Proficiency in working with individuals with disabilities—including developmental disabilities—and managing complex cases involving dementia care or Alzheimer's support.
  • Skilled in crisis management techniques for urgent situations involving mental health crises or trauma.
  • Familiarity with EMR systems for data collection and documentation; experience with inpatient and outpatient clinical settings preferred.
  • Expertise in working with diverse populations such as seniors in geriatrics or post-acute care environments; experience in hospice care or assisted living is advantageous.
  • Knowledge of HIPAA compliance standards along with strong case management skills for individualized patient planning and follow-up.
