Job Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our brand new, hospice care team on a per diem basis. In this vital role, you will provide comprehensive psychosocial support to patients and their families, ensuring they receive holistic care that addresses emotional, social, and mental health needs. Your expertise will help ease the journey during end-of-life care, fostering dignity, comfort, and peace for those we serve. Pay will be PER VISIT to begin with and will be reevaluated once state survey has been completed.
Responsibilities
- Conduct thorough patient assessments to evaluate emotional, social, and behavioral health needs in accordance with hospice care standards.
- Develop personalized care plans that incorporate clinical counseling, behavioral therapy, and family support strategies.
- Provide individual and group psychotherapy sessions focused on grief, adjustment, depression, anxiety, and other mental health concerns related to terminal illness.
- Collaborate with interdisciplinary teams—including nurses, physicians, chaplains, and therapists—to coordinate comprehensive patient care.
- Facilitate crisis intervention and provide immediate support during urgent psychosocial situations or behavioral health crises.
- Maintain accurate medical documentation and records in compliance with HIPAA regulations and healthcare policies.
- Collect data related to patient progress and outcomes for clinical research or quality improvement initiatives.
- Assist with intake procedures for new patients, ensuring seamless integration into the hospice program while respecting patient rights and confidentiality.
Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW or MSW) from an accredited institution; licensure or certification as required by state law.
- Proven experience in hospice care or palliative settings is highly preferred.
- Strong knowledge of medical terminology, ICD coding systems (ICD-10), CPT coding, and electronic medical records (EMR) systems.
- Familiarity with behavioral health modalities such as cognitive behavioral therapy (CBT), motivational interviewing, trauma-informed care (including PTSD), dementia care, Alzheimer’s care, autism spectrum disorders, developmental disabilities, and working with special populations like pediatrics or geriatrics.
- Excellent skills in crisis management, case management, diagnostic evaluation, and mental health counseling techniques.
- Ability to work effectively with diverse populations including children, seniors, individuals with disabilities or chronic illnesses such as dementia or PTSD.
- Strong communication skills for engaging patients and families during emotionally charged situations while maintaining professionalism and empathy.
