Job Summary
We are seeking a compassionate and experienced Medical Social Worker (MSW) to join our healthcare team. The ideal candidate will provide psychosocial support, counseling, and case management services to diverse patient populations across various settings, including inpatient, outpatient, hospice, and community care. This role involves collaborating with medical professionals to enhance patient care and ensure comprehensive support for individuals facing complex health and social challenges. The MSW will utilize clinical expertise to assess patient needs, develop care plans, and facilitate access to resources, ensuring adherence to HIPAA regulations and maintaining accurate medical documentation.
Duties
- Conduct comprehensive patient assessments to identify psychosocial needs, including behavioral health, mental health counseling, and crisis intervention.
- Provide individual, family, and group therapy utilizing techniques such as behavioral therapy, cognitive behavioral therapy, motivational interviewing, and clinical counseling.
- Assist patients with case management services, including coordinating community resources, home & community care, assisted living options, and post-acute care planning.
- Document all interactions thoroughly in electronic medical records (EMR systems), ensuring compliance with HIPAA and other regulatory standards.
- Support patients with specialized needs such as Alzheimer's care, dementia care, PTSD care, autism spectrum disorders, developmental disabilities, and geriatrics.
- Collaborate with interdisciplinary teams on diagnostic evaluation processes using ICD-9/ICD-10 coding and CPT coding when applicable.
- Facilitate intake procedures for new patients and maintain ongoing communication regarding treatment progress.
- Participate in clinical research projects related to behavioral health or social work practices to improve patient outcomes.
- Provide crisis management support and intervene effectively during mental health emergencies or behavioral health crises.
- Maintain professional knowledge of medical terminology, medical records management, and relevant healthcare regulations.
Qualifications
- Master’s Degree in Social Work (MSW) from an accredited institution required.
- Valid state licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent preferred.
- Experience working in hospital settings, hospice care, nursing homes, outpatient clinics, or home & community care environments highly desirable.
- Proficiency in EMR systems and familiarity with ICD coding (ICD-9/ICD-10), CPT coding is advantageous.
- Strong knowledge of HIPAA regulations and experience managing sensitive medical documentation.
- Demonstrated skills in child & family counseling, addiction counseling, crisis intervention, behavioral therapy, and working with people with disabilities or developmental disorders.
- Excellent communication skills for patient assessment, psychotherapy sessions, and interdisciplinary collaboration.
- Ability to handle complex cases involving mental health conditions such as PTSD or dementia while maintaining professionalism under pressure.

Pay: $30.00 - $32.00 per hour
