We have an immediate Opening for a Meds Only Case Manager.
Key Responsibilities:
- This individual will be working directly with children and their families in the community.
- This individual should be familiar with Targeted Case Management and be highly organized to complete assessments, care plans, and monitor the patient's needs.
- Flexible work schedule is required for this position to ensure continuity of care.
Required Skills/Education:
- Bachelors or Master’s Degree in human services, social work, or counseling to work with children 0-18 and their families in the home, community, and school.
- Needs to have experience working with systems, understand family dynamics, behavior management strategies, and work well with a treatment team.
- Strong organizational, liaison, and advocacy skills necessary.
- Prior experience in a mental health, human service, or school setting a plus.
- The Targeted Case Manager must hold a valid driver’s license and be able to demonstrate a clean driving and criminal record.
