-Facilitate day treatment groups

-Develop individualized treatment plans based on assessment findings

- Collaborate with other healthcare professionals to ensure coordinated care for patients
- Monitor and evaluate patient progress throughout the treatment process
- Maintain accurate and up-to-date documentation of patient sessions and progress notes
- Provide case management services to assist patients in accessing necessary resources and support
- Implement behavior management strategies to promote positive changes in patient behavior

```Requirements:```
-3 years experience or degree in a related field
- Strong knowledge of documentation practices
- Excellent communication and interpersonal skills
- Ability to provide patient-centered care with empathy and compassion
- Proficient in data collection and analysis for treatment planning purposes
- Familiarity with case management principles and practices

Pay: $40,000.00 - $57,000.00 per year
