Mental Health Case Manager - Adult Population

This position differs from typical case management in that you will be working with fewer clients, allowing you to provide each individual with the time that they need. Your job duties will include:

• Collaborate with the individual to identify their goals, strengths, and needs
• Monitor the health and welfare of the individual through regular contacts
• Use clinical skills to build rapport and assess mental health status and needs
• Coordinate with the individual, family members, and other service providers
• Advocate for access to resources and continuity of services
• Assess adequacy of service provision and address shortfalls or barriers
• Identify changes in the individual’s situation or needs

As contractors with the Texas Health and Human Services Commission (HHSC), we provide case management services to individuals with serious mental illness so that they may live in the community and thrive. Individuals enrolled in this HHSC program are provided a broad array of services for as long as they need them, including housing, transportation, and nursing.

Job Requirements:

• A master’s degree in human services or a related field
• Bilingual in Spanish and English
• 2+ years of experience working with adults with serious mental illness
• Ability to take initiative and to adapt to changing circumstances
• Ability to work collaboratively with others and find creative solutions
• Ability to earn trust, be accountable, and meet commitments

Work location:

• You will meet with clients where they live (mostly at supervised living homes) and at their appointments
