- Manage discharge planning processes by preparing comprehensive medical records, facilitating post-acute care arrangements, and ensuring compliance with HIPAA regulations.
- Engage in utilization management by reviewing diagnostic evaluations, managing patient intake procedures, and overseeing case management for inpatient and outpatient services.
- Provide addiction counseling support when appropriate and assist in managing complex cases involving behavioral health or mental health disorders.
- Maintain detailed documentation of patient interactions, medical records, and case notes while adhering to confidentiality standards and hospital policies.
Skills
- Strong knowledge of managed care processes, discharge planning procedures, and utilization management strategies.
- Proficiency in medical terminology, physiology knowledge, patient assessment techniques, and diagnostic evaluation methods.
- Experience working with behavioral health populations in outpatient clinics; familiarity with addiction counseling is a plus.
- Excellent communication skills to coordinate effectively with multidisciplinary teams including social workers and healthcare providers.
- Ability to navigate complex medical records systems while maintaining HIPAA compliance; experience with outpatient or post-acute care settings is advantageous.
