Job Overview
The Mental Health Scribe supports psychiatric providers by documenting clinical encounters in real time and preparing all necessary paperwork to ensure efficient, compliant, and high-quality patient care. This role assists with daily chart preparation, assessment documentation, filing, and auditing to maintain accurate medical records and
meet regulatory standards.
Clinical Documentation
- Document patient encounters in real time during psychiatric evaluations, follow-ups, and assessments.
- Accurately enter history of present illness (HPI), mental status exams (MSE), treatment plans, and provider recommendations into the EHR.
- Ensure all clinical documentation reflects provider direction and meets compliance standards.
- Assist with completion of prior authorizations, referral forms, and other required documentation.
Daily Chart Preparation
- Prepare and organize charts prior to scheduled appointments.
- Ensure required forms, consents, and assessments are uploaded and available for providers.
- Review charts for missing documents or unsigned notes and flag discrepancies.
Filing & Record Management
- Complete daily electronic filing of documentation in appropriate patient records.
- Maintain organized and up-to-date digital records in accordance with HIPAA guidelines.
- Assist with scanning, uploading, and indexing documents accurately.
Qualifications
- High school diploma required; associate or bachelor’s degree in healthcare, psychology, or related field preferred.
- Previous scribe, medical assistant, or behavioral health documentation experience preferred.
- Strong understanding of HIPAA and confidentiality standards.
- Proficiency in Electronic Health Record (EHR) systems.
- Excellent typing speed and attention to detail.
- Ability to maintain accuracy in a fast-paced clinical environment.
