Schedule: Monday–Friday, 8:30 AM–5:00 PM (No weekends or after-hours!)

For over 40 years, Douglas Gardens Community Mental Health Center (CMHC) has proudly served the Miami-Dade community with high-quality, compassionate mental health services. We are now seeking a dedicated full-time Targeted Case Manager to join our North Miami Beach team — help clients living with mental illness access the support and services they need to lead more stable, independent lives.

What You’ll Do:

As a Targeted Case Manager, you’ll support adults living with mental illness by helping them access the services and resources they need to thrive. Your role will include:

• Conducting comprehensive case management assessments to understand client needs.
• Creating individualized service and treatment plans.
• Linking clients with vital community resources, providers, and supports.
• Monitoring progress and adjusting plans as needed.
• Assisting with prior authorizations for services or medications.
• Completing required documentation and reports
• Conducting home and community visits as required
• Advocating to ensure clients receive necessary care and support.

What We’re Looking For:

• Bachelor’s degree in Social Work, Psychology, Criminal Justice, or a related human services field (from an accredited college/university).
• At least one year of experience in a similar role is preferred
• Bilingual in English/Creole.
• Strong communication, organizational, and problem-solving skills.
• Valid Florida Driver’s License and automobile insurance required.
• Pass Level 2 background check and drug screening
• Complete AHCA Targeted Case Management training within 3 months of hire (we provide it!)
• Commitment to attaining Florida Certification Board’s Certified Behavioral Health Case Manager (CBHCM) certification within 6 months (we reimburse the application fee!)

Pay: $40,000.00 - $45,000.00 per year
