Mental Health Targeted Case Manager

Overview
Provide Targeted Case Management services to assigned clients. Services may include: assessments; service planning; coordination, linkage and monitoring of mental health progress and needs; crisis intervention; maintenance of medical records and documentation.

b. Maintain demonstrated proficiency in child and/or adult oriented mental health and case management skills by submitting documentation of continuing education in these areas.

c.Act in a competent and professional manner in carrying out the duties of his/her the term of the agreement

d.Complete all required documentation in a timely manner and ensure maintenance of complete clinical record at all times for each assigned client, including but not limited to:

i. Completing assessments for each new client within required timelines and updating assessments on an annual basis,

ii.Completing initial and subsequent Individual Service Plans within required timelines,

iii.Documenting and filing progress notes within 7 days of each billable service provided

iv. Obtaining and maintaining accurate client personal data, consents, and relevant collateral documentation.

v.Other politics of the company related to the Independent Contractor’s performance

e.The Contractor will complete and enter clinical and billing documentation into EHR Notes System within 48 business hours of providing the services. Necessary corrections must be made 48 hours after receiving the recommendations. Fail to do so may affect payment services.

Pay: $24.00 per hour

• Bachelor's (Preferred)

License/Certification:

• Florida Board Certification (Required)
