Clinical & Service Responsibilities
- Provide community-based behavioral health support services to members with mental health, substance use, and psychosocial needs
- Complete comprehensive assessments, service notes, and person-centered documentation
- Assist members with goal development, skill-building, crisis prevention, and community integration
- Coordinate care with providers, therapists, case managers, referral sources, and outside agencies
- Monitor member progress toward treatment goals and provide ongoing support and interventions
- Conduct face-to-face visits in office, home, school, hospital, shelter, or community settings as assigned
- Provide psychoeducation, coping skills training, and support with independent living skills
- Assist members with accessing housing, medical care, transportation, employment, and community resources
- Maintain productivity expectations and complete documentation within required timeframes
- Participate in treatment team meetings, staffing, supervision, and trainings
Administrative & Operational Responsibilities
- Complete timely and accurate documentation within the Electronic Health Record (EHR)
- Maintain daily schedules, appointments, and member contact logs
- Coordinate referrals and follow-up appointments
- Communicate updates and concerns to supervisors and treatment team members
- Ensure compliance with Medicaid, state, and agency documentation requirements
- Assist with audits, authorizations, and record reviews as requested
- Enter and maintain member notes
- Complete and review QP staff notes
- Check and respond to agency emails daily
- Complete initial supervision plans for staff and communicate updates to Human Resources
- Complete initial Person-Centered Plans (PCPs) and Crisis Plans and submit to Medical Records
- Review staff caseloads and update authorization tracking lists
- Participate in administrative meetings and staff caseload meetings
- Review grievances and incident reports as needed
- Monitor non-maximized service hours and productivity
- Participate in team meetings with core staff and treatment teams
- Complete documentation and updates within Microsoft systems and internal tracking platforms
- Attend monthly staff meetings and mandatory agency meetings
- Participate in chart audits and quality assurance activities
- Conduct quality insurance and compliance calls
- Update PCPs, Crisis Plans, PHQ-9, and GAD-7 tools as required
- Maintain and update community resource lists
- Participate in peer review activities with leadership
- Track therapy and medication management appointments
- Process member discharges for no-show status, inactivity, or as clinically indicated
- Participate in supervision sessions and staff development activities
- Track treatment compliance for treatment team meetings and required assessment tools
- Complete NC TOPPS documentation and updates
- Schedule and coordinate documentation updates
- Complete annual trainings and compliance requirements
- Update intake documentation and admissions records
- Complete grievance and incident reporting documentation
- Create and maintain staff supervision plans
- Participate in disciplinary action processes as directed by leadership
- Complete DLA-20 assessments and updates
Communication & Coordination
- Communicate effectively with members, families, providers, hospitals, DSS, schools, and community agencies
- Maintain professional communication consistent with HIPAA and 42 CFR Part 2 requirements
- Serve as a liaison between members and treatment providers to support continuity of care
- Respond professionally and appropriately during crisis situations
- Maintain healthy professional boundaries with members and families
Compliance & Professional Standards
- Maintain strict confidentiality of protected health information
- Follow all agency policies, procedures, and professional conduct standards
- Participate in staff meetings, trainings, supervision sessions, and continuing education
- Adhere to HIPAA, OSHA, infection control, and workplace safety standards
- Demonstrate professionalism, empathy, cultural sensitivity, and trauma-informed care practices
- Maintain accurate and timely service documentation according to Medicaid standards
Qualifications
Education
- Bachelor’s degree in Human Services or related field required

OR
- Qualified Professional status meeting North Carolina Medicaid and DHHS requirements
Experience
- Minimum 1 year of experience working with individuals with mental health, substance use, or behavioral health needs preferred
- Community-based service experience preferred
- Experience working with Medicaid populations preferred
- Experience with Electronic Health Records (EHR) preferred
- Knowledge of person-centered planning and behavioral health documentation preferred
- Experience with treatment planning, crisis planning, and service coordination preferred

Preferred Qualifications
- Experience with NC Medicaid services
- Knowledge of Trillium, Vaya, Alliance, Partners, or Healthy Blue processes
- Experience working with individuals diagnosed with severe mental illness and substance use disorders
- Knowledge of trauma-informed care and recovery-oriented treatment approaches
- Valid North Carolina driver’s license and reliable transportation
- Familiarity with PHQ-9, GAD-7, DLA-20, and NC TOPPS requirements
Physical Requirements
- Ability to sit, stand, bend, and travel throughout the community as needed
- Frequent computer, phone, and documentation use
- Ability to transport and assist with member support activities
- Ability to lift up to 15–25 pounds occasionally
- Clear verbal and written communication skills
Work Environment
- This position includes office-based and community-based work settings serving individuals with mental health, substance use, and psychosocial needs.
- Qualified Professional (QP) will work closely with leadership, providers, therapists, peer support staff, and community agencies to support quality member care and continuity of services.
