Job Overview
We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive primary care services within a School-Based Health Center (SBHC) operated by our Federally Qualified Health Center (FQHC). The provider will deliver age-appropriate care to students, promote wellness, and collaborate with school staff, families, and healthcare teams to improve student health outcomes.
Key Responsibilities:
- Provide primary care services including assessment, diagnosis, and treatment of acute and chronic conditions
- Perform physical exams, well-child visits, and preventive health screenings
- Administer immunizations and manage medication therapies
- Promote health education and disease prevention among students
- Maintain accurate and timely electronic medical records (EMR) documentation
- Collaborate with school personnel, behavioral health providers, and community resources
- Participate in quality improvement initiatives and FQHC compliance requirements
- Ensure adherence to federal, state, and organizational regulations
Qualifications:
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current, unrestricted WV license
- Board certification (e.g., AANP, ANCC, NCCPA)
- DEA license (or eligibility)
- Experience in pediatrics, family medicine, or school-based care preferred
- Experience in an FQHC or underserved setting is a plus
- Certification in Basic Life Support (BLS) required

Pay: $90,000.00 - $120,000.00 per year
