Mid-Level Practitioner - Summersville School Based Health Center

Job Overview
We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide comprehensive primary care services within a School-Based Health Center (SBHC) operated by our Federally Qualified Health Center (FQHC). The provider will deliver age-appropriate care to students, promote wellness, and collaborate with school staff, families, and healthcare teams to improve student health outcomes.

Key Responsibilities:

  • Provide primary care services including assessment, diagnosis, and treatment of acute and chronic conditions
  • Perform physical exams, well-child visits, and preventive health screenings
  • Administer immunizations and manage medication therapies
  • Promote health education and disease prevention among students
  • Maintain accurate and timely electronic medical records (EMR) documentation
  • Collaborate with school personnel, behavioral health providers, and community resources
  • Participate in quality improvement initiatives and FQHC compliance requirements
  • Ensure adherence to federal, state, and organizational regulations

Qualifications:

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program
  • Current, unrestricted WV license
  • Board certification (e.g., AANP, ANCC, NCCPA)
  • DEA license (or eligibility)
  • Experience in pediatrics, family medicine, or school-based care preferred
  • Experience in an FQHC or underserved setting is a plus
  • Certification in Basic Life Support (BLS) required

Pay: $90,000.00 - $120,000.00 per year
