Midlevel Clinical Care Coordinator

Unique opportunity to provide evidence-based and outcome-focused care coordination for patients and internal treatment teams. As part of a trauma-trained multidisciplinary care team, this individual will coordinate care to improve or maintain mental, emotional, social, functional, and physical health of patients. The Care Coordinator will also offer professional consultation and education to team members and patients, as well as provide patients with advocacy, linkage, and care coordination with other health or social service agencies in the community as needed. Assist in development of integrated treatment planning and intervention development.

Responsibilities:

  • Complete NOMS and various health screenings and assessments.
  • Work collaboratively with treatment teams to decrease hospitalization, ED utilization, and reduce overall risk.
  • Consult with agency staff including psychiatrists, primary care providers, addiction specialists, behavioral health treatment teams.
  • Assess treatment patterns/utilization of services and coordinate care/referrals with interdisciplinary team.
  • Develop and implement care plans in EMR, evaluate effectiveness, monitor in a timely way, and effect changes as needed.
  • Follow up on clinical referrals.
  • Help improve, measure, monitor and sustain quality outcomes.
  • AA/BA/CHW required; CHW, LPN or LSW/LPC strongly preferred.
  • Experience with SPMI and/or SUD population preferred.
  • Strong interpersonal and communication skills.
  • Ability to work effectively as part of an interdisciplinary team.
  • Crisis intervention and de-escalation skills.
  • Knowledge of community resources and healthcare services.
  • Valid driver's license and insurance.
