Unique opportunity to provide evidence-based and outcome-focused care coordination for patients and internal treatment teams. As part of a trauma-trained multidisciplinary care team, this individual will coordinate care to improve or maintain mental, emotional, social, functional, and physical health of patients. The Care Coordinator will also offer professional consultation and education to team members and patients, as well as provide patients with advocacy, linkage, and care coordination with other health or social service agencies in the community as needed. Assist in development of integrated treatment planning and intervention development.
