Job Summary
We are seeking an experienced and highly accountable MDS Coordinator (LVN or RN) to join our 50-bed skilled nursing facility. The ideal candidate will have at least two (2) years of MDS experience, strong PDPM knowledge, and the ability to take full ownership of the MDS process from start to finish. MDS Coordinator will take full ownership of the MDS and care planning process, not just assessments
This role is critical to ensuring accurate assessments, timely submissions, regulatory compliance, and appropriate reimbursement. We are looking for a self-directed professional with excellent assessment skills, strong clinical judgment, and a deep understanding of Medicare and Medi-Cal requirements.
Key Responsibilities
- Maintain full responsibility and ownership of the MDS process, including scheduling, completion, transmission, and corrections
- Complete and coordinate all required MDS assessments (OBRA, PPS, significant change, quarterly, annual, and discharge) in a timely and compliant manner
- Perform thorough resident assessments and ensure accurate coding to support PDPM reimbursement optimization
- Collaborate with the interdisciplinary team (IDT) to ensure clinical documentation supports resident status, care planning, and reimbursement
- Ensure compliance with CMS, Medicare, Medi-Cal, and state regulatory requirements
- Review clinical documentation for accuracy, completeness, and consistency with MDS coding
- Utilize PointClickCare (PCC) and other applicable systems for documentation and submissions
- Monitor ARD schedules, assessment timelines, and submission deadlines
- Participate in QAPI activities, audits, and survey readiness related to MDS and documentation
- Provide education and guidance to nursing and IDT staff regarding MDS, PDPM, and documentation requirements
- Maintain up-to-date knowledge of changes in PDPM, MDS guidelines, and CMS regulations
Qualifications & Requirements
- Active LVN or RN license in the state of California
- Minimum of 2 years of experience as an MDS Coordinator in a skilled nursing facility
- Strong working knowledge of PDPM and MDS 3.0
- Proven ability to independently manage the entire MDS process
- Experience with PointClickCare (PCC) strongly preferred
- Excellent clinical assessment skills and attention to detail
- Strong organizational and time-management abilities
- Ability to work independently while collaborating effectively with an interdisciplinary team
- Strong communication skills and professional accountability
Job Type: Full-time
Pay: $93,500.00 - $124,000.00 per year
