Nurse Practitioner – Genesee & Lapeer County (Pipeline Role)

HomeMD delivers mobile primary care services, enabling Nurse Practitioners to provide in-home medical care to senior patients in residential and community-based environments. This approach enhances access to care while minimizing the need for patient travel.
Providers generally operate within an approximate 45-mile radius from their home location, with only occasional travel beyond this range.
KEY RESPONSIBILITIES
Providing In-Home Clinical Services
- Conduct thorough patient assessments within home settings
- Monitor vital health indicators and recognize changes in condition
- Revise and update care plans based on clinical evaluations
Managing Chronic Conditions
- Deliver care for patients with ongoing conditions such as heart disease, diabetes, and cognitive impairments
- Maintain consistency and continuity in treatment plans
Medication Management
- Review existing medications for appropriateness and effectiveness
- Coordinate with pharmacies and other healthcare providers for any adjustments
- Educate patients and caregivers on correct medication usage
Enhancing Patient Quality of Life
- Support care strategies focused on comfort and symptom relief
- Collaborate with patients and families managing long-term or advanced conditions
Preventive Care
- Conduct routine health screenings, immunizations, and wellness visits
- Promote preventative measures to reduce health risks
Timely Care Response
- Address urgent concerns through same-day or remote evaluations
- Work with the care team to coordinate timely support when needed
Care Coordination
- Collaborate with interdisciplinary teams, including physicians and facility personnel
- Maintain accurate, up-to-date patient documentation
ROLE IMPACT
- Improves healthcare access for underserved senior populations
- Enables early detection of health issues, reducing hospital visits
- Supports continuity of care through consistent follow-ups
- Provides families with confidence through reliable medical oversight
COMMON SERVICES PROVIDED
- In-home primary care visits
- Management of chronic conditions
- Behavioral health check-ins with available support resources
- Fall risk and prevention evaluations
- Coordination of basic wound care and referrals
- Ordering laboratory tests, imaging, and other diagnostics
WORK ENVIRONMENT & SUPPORT
Scheduling and logistics are handled by a centralized coordination team. Providers receive remote administrative support, allowing them to focus more on delivering patient care.
QUALIFICATIONS
- Active Nurse Practitioner license in Michigan
- Master’s or Doctorate degree in Nursing with board certification
- Prior experience in geriatrics, home health, or complex care settings preferred
- Ability to work independently in a field-based environment
- Strong communication skills and sound clinical judgment
IDEAL CANDIDATES
Candidates with experience as a Nurse Practitioner (NP), Advanced Practice Provider (APP), Family Nurse Practitioner, Adult-Gerontology NP, or similar roles
