Suru PT & OT provides in-home outpatient rehabilitation across Southern California. We bring high-quality PT/OT services directly to patients at home, with strong operational support so clinicians can focus on care.
Why this role
- We handle virtual eval + billing/admin so you can focus on patient care and visits.
- We provide steady in-home outpatient (Part B) visits when patients are in your coverage area.
- We support a discharge referral loop: if you identify home health discharges, send them to us, we complete telehealth evals, and route patients back for ongoing in-home outpatient visits.
Responsibilities
- Provide in-home treatment visits as a PTA or OTA based on supervising therapist plan of care
- Deliver skilled therapeutic interventions to improve function, safety, mobility, and ADL performance
- Progress treatment plans and report changes in condition to supervising PT/OT
- Educate patients/caregivers on home program and safety strategies
- Document visits accurately and on time
- Coordinate scheduling and care continuity with operations team
Required qualifications
- Active California PTA or OTA license in good standing
- Reliable transportation and willingness to travel within your selected coverage area in Los Angeles County
- Current CPR/BLS
- Strong communication and patient-centered care skills
- Comfortable with timely documentation and mobile care workflow
- Professional, dependable, and responsive
Onboarding / compliance requirements
- Valid professional license documentation
- business license (required for California contractor onboarding)
- Ability to complete credentialing/background requirements as applicable
Job Types: Part-time, Contract
Pay: $60.00 - $75.00 per hour
