Mobile Wound Care Nurse Practitioner

We are seeking a compassionate and skilled Nurse Practitioner to join our mobile wound care team and play a critical role in improving patient outcomes through specialized wound management.

Job Summary:

The Nurse Practitioner (NP) – Mobile Wound Care will be responsible for assessing, diagnosing, and managing acute and chronic wounds in a variety of patient settings. The NP will provide direct patient care, collaborate with interdisciplinary teams, and develop individualized treatment plans to promote healing and prevent complications. This role requires a high level of autonomy, clinical expertise, and the ability to travel to patient locations.

Key Responsibilities:

- Conduct comprehensive wound assessments, including wound staging, measurements, and risk factor analysis.

- Develop and implement individualized treatment plans using evidence-based wound care protocols.

- Perform advanced wound care procedures, including debridement, dressing changes, and negative pressure wound therapy as needed.

- Order and interpret diagnostic tests related to wound care management.

- Prescribe medications, dressings, and treatment regimens within the scope of practice.

- Collaborate with primary care providers, specialists, nurses, and facility staff to coordinate care and ensure optimal patient outcomes.

- Educate patients, caregivers, and facility staff on wound prevention, treatment, and best practices.

- Maintain accurate and timely documentation in the electronic health record (EHR) system.

- Travel to various patient locations within the designated service area. Adhere to infection control protocols, HIPAA regulations, and company policies.

Qualifications & Skills:

- Master’s or Doctorate degree in Nursing (MSN/DNP) from an accredited program. Active Nurse Practitioner (NP) license in the state of practice.

- Board Certification as an NP in Adult-Gerontology, Family, or Wound Care preferred.

- Minimum of 1-2 years of wound care experience (preferred) or strong interest in specializing in wound management.

- Certification in Wound Care (WCC, CWON, or CWCN) preferred but not required. Strong knowledge of wound care treatments, products, and therapies. Experience working in home health, skilled nursing, or long-term care settings is a plus.

- Ability to work independently and manage a mobile patient caseload. Excellent communication, problem-solving, and organizational skills. Willingness to travel within the designated service area (valid driver’s license required).

Pay: $110,000.00 - $140,000.00 per year
