Role & responsibilities
- Maintain, organize and manage patients' medical records in both physical and electronic formats.
- Ensure accuracy, completeness and timely updating of medical records as per hospital policies.
- Retrieve and file patient records efficiently to support clinical and administrative needs.
- Code, index, and archive medical records according to standard classification systems.
- Ensure confidentiality and security of patient information in compliance with legal and ethical standards.
- Coordinate with doctors, nurses, and other departments for completion of documentation.
-Monitor and track missing, incomplete, or pending medical records and follow up for closure.
-Handle medico-legal cases (MLC) records as per regulatory requirements.
- Maintain discharge summaries, consent forms, and investigation reports properly.
- Generate MIS reports related to admissions, discharges, and medical records as required
Preferred candidate profile
-Knowledge of medical terminology, documentation, and hospital workflows.
-Familiar with HIS/EMR systems and MS Office
-Understanding of NABH standards and patient data confidentiality
-Good coordination, organizational, and communication skills
