MSW/ Social Worker

MSW/ Social Worker Responsibilities:

• Intake screen (Check eligibility, fill out intake form, provide tour, etc.)
• Conduct initial assessments, bi-annual reassessments.
• Conduct initial home evaluations and home visits as needed.
• Individual and group counseling to elderly participants
• Connect participants with supporting resources, such as IHSS and Access Paratransit.
• Provide case management.
• Complete quarterly assessment, progress notes, flow sheets in a timely manner.
• Planning and coordination of discharges and manage discharge folders.
• Bilingual (spoken and/or written) in Chinese (Mandarin and/or Cantonese)
• Rotating Saturdays.
• H1b sponsorship available if qualify.

Job Type: Full-time

Pay: $25.00 - $30.00 per hour
