Position: Lead Nephrologist

Position Summary

Our client is seeking a dedicated Lead Nephrologist to provide high-quality, value-based care to patients with chronic kidney disease (CKD), end-stage renal disease (ESRD), and other complex conditions. This full-time role involves both in-person care (including home visits) and telehealth services, offering a unique opportunity to practice in a patient-centered, innovative model of kidney care.

Key Responsibilities

  • Deliver comprehensive nephrology care across diverse settings, including patient homes, telehealth, and dialysis centers.
  • Serve as a clinical leader, guiding patient care strategies and supporting collaborative decision-making with multidisciplinary teams.
  • Develop and oversee individualized care plans that improve patient outcomes and align with value-based care initiatives.
  • Engage patients and families in shared decision-making to promote health literacy and self-management.
  • Contribute to quality improvement initiatives and population health management strategies.

Qualifications

  • MD or DO degree from an accredited medical school.
  • Board Certification in Nephrology.
  • Active, unrestricted state medical license and DEA registration.
  • Proven experience managing patients with CKD, ESRD, and other kidney-related conditions in inpatient, outpatient, and dialysis settings.
  • Strong clinical judgment and commitment to patient-centered care.
  • Excellent communication and interpersonal skills to collaborate effectively with patients, families, and healthcare teams.
  • Experience in value-based care, quality improvement, or population health preferred.

Compensation & Benefits

  • Competitive base salary plus performance-based bonus program.
  • Full-time schedule: Monday–Friday, 8:30 AM – 5:00 PM.

Job Type: Full-time

Pay: $250,000.00 - $300,000.00 per year
