Overview: We are seeking a dedicated Next-Gen Care Coordinator (NGCC to join our healthcare team. The Next-Gen Care Coordinator (NGCC) is a cardiovascular-focused clinical position designed to support CardioElite’s advanced care delivery model across skilled nursing facilities (SNFs). This hybrid role combines direct patient interaction, diagnostic testing, and care coordination, bridging the gap between bedside care and advanced cardiovascular management. NGCCs are key members of each CardioPod team, ensuring seamless communication between providers, facilities, and the CardioElite operations infrastructure..

Core Responsibilities

· Clinical & Diagnostic

  • Conduct AI-assisted point-of-care echocardiograms, 12-lead EKG testing, and other ancillary cardiovascular testing as clinically indicated.
  • Work closely with the practice and providers to properly identify and manage patients who may benefit from Chronic Care Management (CCM) programs.
  • Optimize and utilize CPT codes as part of the documentation and workflow process (not as the responsible billing party, but as an active participant supporting accurate coding).
  • Ensure appropriate documentation, data capture, and timely upload to the EMR and CardioElite analytics systems.

· Operational Support

  • Coordinate daily rounds and testing schedules with assigned ARNPs or physicians.
  • Manage logistics, equipment readiness, and facility coordination.
  • Serve as the communication link between facility staff, CardioElite providers, and the central operations team.
  • Support onboarding and ongoing engagement of new facilities under the CardioElite program.

· Technology & Integration

  • Operate AI-enabled diagnostic platforms for testing and reporting.
  • Use digital dashboards to monitor productivity metrics and patient tracking.
  • Support implementation of ancillary service programs (EKG, AISAP, CCM, and others).
  • Participate in the ongoing evolution of the CardioElite technology ecosystem, including AI-driven workflow tools.ns:

Performance Expectations

  • Assigned to one or more facilities, rotating within a defined territory.
  • Target output: 30 to 40 EKGs per day, depending on facility size and case mix.
  • Maintain >95% documentation accuracy and completion daily.
  • Demonstrate professionalism, reliability, and strong communication with DONs, administrators, and providers.
  • Uphold CardioElite’s mission to improve cardiovascular outcomes, reduce readmissions, and strengthen post-acute care coordination

Qualifications

  • Certified Medical Assistant or equivalent clinical experience preferred.
  • Familiarity with cardiovascular testing and terminology.
  • Comfort using mobile technology, EMRs, and AI diagnostic tools.
  • Excellent communication, teamwork, and organizational skills.
  • Reliable transportation; mobile role within assigned counties (Miami-Dade, Broward, Palm Beach).

Compensation Framework

  • $3,000 to $4,000 per month (4 to 6 hours per day)
  • Incentive: Tiered performance bonuses based on completed tests, CCM enrollments, and AISAP completions
  • Total Annual Bonus
  • Additional Benefits: Mileage reimbursement and/or car allowance, CCM enrollment bonuses, quarterly recognition, and eligibility for NGCC II Leadership Track.
