Non-Medical Case Manager (RN)

Visiting Angels of North Jax is seeking a compassionate and experienced RN to join our growing home care team. WE NEED SOMEONE PROFESSIONAL, COMPUTER SAVVY AND UNDERSTANDS URGENCY, AS THIS IS A FAST-PACED POSITION. This role leads our Veteran Referral Division and serves as a key connection between clients, families, caregivers, and healthcare professionals. Develop and coordinate individualized, person-centered care plans for clients with a variety of needs (e.g., dementia, Alzheimer’s, stroke, amputees, loss of sight, PTSD...)

• Receive and manage referrals and client inquiries for Veteran-related services
• Assist with initial caregiver scheduling and care setup
• Train and evaluate caregivers on required skills and care standards
• Supervise caregiver performance and communicate feedback to Scheduling Managers and the Director of Operations
• Collaborate as needed with physicians and other healthcare professionals regarding care needs and changes
• Ensure compliance with HIPAA and agency documentation standards

Qualifications

• Active FL. RN License.
• Minimum 3 years of nursing experience (home care, hospital, long-term care, assisted living, or similar settings)
• BLS certification required
• Experience working with geriatric populations, dementia care, hospice, or Veterans strongly preferred
• Familiarity with ICD-9 coding is preferred
• Strong communication skills with a compassionate, professional approach
• Ability to work independently while managing your schedule efficiently.

Background Screening Requirement

In accordance with Florida House Bill 531, all candidates must complete a Level 2 background screening through the Florida Clearinghouse / Agency of Health Care Administration (AHCA) as a condition of employment.
