Duties

• Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
• Monitor patients’ vital signs and observe for any changes in health or behavior
• Following care plans and HIPAA regulations to ensure patient confidentiality
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
• Support clients during outings or community activities to promote social engagement
• Operate medical equipment like Hoyer lifts or wheelchairs safely and effectively
• Maintain detailed documentation of care provided, patient observations, and any incidents or concerns

Skills

• Proven experience in home health, senior care, or long-term care environments
• Knowledge of first aid, CPR, and emergency response procedures
• Familiarity with Alzheimer’s care, dementia care, memory care, and working with individuals with developmental disabilities
• Ability to assist with medication administration and vital signs monitoring accurately
• Strong communication skills for effective interaction with clients, families, and healthcare teams
• Physical stamina for heavy lifting and assisting with mobility tasks
• Understanding of HIPAA regulations to protect patient privacy and confidentiality
